Dear Golf Club Member,
It’s that time of year again! Jerry Knowlton has again made arrangements for our club members to purchase a little peace of mind by becoming a Life Flight member through Mt. Hood Golf Club. A Life Flight Network Membership relieves you from liability for out-of-pocket costs of emergent, medically necessary transport to the closest, most appropriate facility not only for you but for all eligible household members. Please carefully read the Life Flight Statement of Understanding (download on this page) which discusses all program details including who is covered under your membership. As in years past, you are getting a special MHGC group discount of $75.00. By submitting your payment, you are agreeing that you have read and understand the Statement of Understanding. Life Flight Enrollment Forms (download on this page) and Payment Instructions For Current Life Flight Members If you completed an Enrollment Form last year, and no information has changed, you do not need to complete another form for this year. You can simply send your $75.00 to: Mt. Hood Golf Club, P.O. Box 26, Welches, OR 97067. Or you can make your payment on this page. If you completed an Enrollment Form last year but you have an updated address, name correction and/or a new household member (date of birth required), please complete a new Enrollment Form on this page and mail it to: Mt. Hood Golf Club, P.O. Box 26, Welches, OR 97067 along with your payment of $75.00. Or you can make a credit card payment on this page. For New Life Flight Members If you are new to the Life Flight Program, please complete the Enrollment Form on this page and mail it to: Mt. Hood Golf Club, P.O.
